Giant hernias have chronic abdominal wall defects that slowly and progressively grow alter the normal physiology of wall. The clinical case we report is a 76-year-old male who began 7 years ago with protrusion in left inguinal area progressive increase volume until reaching inability to walk presented. protocol followed by pneumoperitoneum insufflation 500-1000 milliliters (ml) carbon dioxide daily for 10 days. treatment large hernia produces intra-abdominal pressure during visceral reduction, which entails an alteration cardiorespiratory biomechanics elevating diaphragm reducing venous return. This technique would be indicated greater than centimeters (cm), as well umbilical hernias, irreducible sacs, "loss domain". It protocolized preoperative preparation days, subsequent plasty Lichtenstein technique, minimum adhesions were found, managing preserve ipsilateral testicle, surgical time hospital stay, no data on compartment syndrome abdomen after surgery, providing good alternative this type condition.
